The community gateway is a new portal that will provide coordinated help and support from the right ‘bit’ of the voluntary sector.
It is now open from 8am till 8pm, 7 days a week and is staffed by friendly, knowledgeable people who will receive referrals via email and take phone calls and arrange for follow up visits and support where necessary.
The team will listen to people’s story then discuss their options with them and them get support from the right voluntary and community sector partner.
This will involve, if required a face-to-face personalised conversations to develop personalised/anticipatory care and support plans.
