Blood Glucose Monitoring

Thank you for visiting this page to submit your blood glucose readings to the diabetes team at Oak Tree Surgery and Pensilva Health Centre.

Please add your blood glucose readings for the times that you have. Not everyone checks their blood glucose levels at all of the times below. Pre-meal readings, particularly before breakfast are very helpful to us. Your clinician may have asked you to take readings at some extra times in the day/night.

For more information on how to take your blood glucose levels, please visit Blood Glucose Testing.
